- Download the VISA® Debit Card application and print it on your printer.
- Complete the VISA® Debit Card Application (be sure to read the VISA® Debit Card Agreement & Disclosure Statement).
- Mail the VISA® Debit Card Application (third page) to us and leave the VISA® Debit Card Agreement & Disclosure Statement for your records.
- Currently only accepting applications for Credit Union members only.
