Membership sign-up instructions
- Download the Membership Application Card and print it on your printer.
- Complete the Membership Application Card (be sure to read and complete two parts of it).
- Sign-up in Person: Bring cash, check or money order for your initial deposit of $56.00 ($1.00 - new account fee, $5.00 - membership fee) payable to
Ukrainian National Federal Credit Union with your completed Membership
Card.
- Copies of appropriate documentation required with completed application
such as Social Security, Taxpayer Identification Number, Passport, Valid photo
ID, etc.
