• Download the Mortgage Packet and print it on your printer (six pages). Use a legal paper size 8.5x14in.
• Complete the Borrower's Certification & Authorization form (one page)
• Complete the Disclosure Notice (one page)
• Complete the Uniform Residential Loan Application (four pages)

Once you have completed your mortgage application you must bring the following items to our office for your initial mortgage interview:

• Fully completed Uniform Residential Loan Application
• Signed Borrower's Certification and Authorization
• Signed Disclosure Notice
• Driver's License
• Social Security Card
• Income Tax Returns (two years for employed by others, three - for self-employed)
• Two (2) years W-2 Forms (all employers)
• Current thirty (30) day pay stubs
• Copy of purchase Contract (if applicable)
• Copy of paid real estate tax receipt (if applicable)
• Copy of Hazard Insurance Policy (if applicable)
